Miner Falls Through Prep Plant
Floor
On September 10, 2002, a fall-of-person accident occurred at a
preparation plant located in District 8. A plant
operator/repairman was washing down the fourth floor of the
preparation plant. He was using a large pressurized hose with a
nozzle. While spraying with the hose, he stepped on an expanded
metal grate that had been partially removed from its proper
position to allow an air hose to be lowered to the floor below.
When he stepped on the grate, the grate and the miner fell 11
½ feet to the floor below. He sustained severe lacerations
and possible head injuries.
BEST PRACTICES
- Keep covers for floor openings securely in place.
- Wear a properly secured safety harness and tag line when
there is danger of falling.
- Get help with high-pressure water lines.
