COAL MINE FATALITY - On January 9, 1995, at 10:45 a.m., a contractor employee was fatally injured as a result of a fall in a preparation plant. The victim was using a set of come-alongs to hoist a metal distribution box from the second floor to the third floor. As the box was being swung toward the victim, he slipped off a cross member and fell through the open refuse chute to the concrete floor, 27 feet below. He died as a result of the injuries on January 15.

This is the third coal mine fatality in 1995. As of January 9 last year, there had been one fatality. This death is the first classified as FALL OF PERSON in 1995.
