COAL MINE FATALITY - On July 27, 1995, at 10:00 a.m., a delivery truck driver was fatally injured while removing the highway tie-down straps on a load plastic pipe. The truck had been loaded with ten bundles of nine pipes each. The individual bundles were secured with four steel bands. Upon the removal of the four main-load nylon tie-down straps, the top bundle's steel bands broke. The plastic pipes, apparently under tension, shot out from the truck passing over the truck driver. A second bundle, weighing 3,000 pounds, then fell striking the truck driver causing fatal injuries. This is the 23rd coal mine fatality in 1995. As of July 27 last year there had been 27 fatalities. This death is the first death classified as FALL OF MATERIAL in 1995.  Line drawing of the accident sceneFor more information: |
