METAL/NONMETAL MINE FATALITY - On August 3, 1995, a loader operator with 10 years of mining experience was killed at a sand and gravel operation. The employee was operating a front-end loader hauling material from the gravel bank to the edge of the 16-foot high pit. It appears that as the loader approached the edge of the pit, the wall sloughed off causing the loader to turn upside down into the pit and crush the victim. The victim died from sustained injuries. This is the 23rd fatality reported in calendar year 1995 in the metal and nonmetal mining industries. As of this date in 1994, there were 28 fatalities reported in these industries. This death is the 10th fatality classified as powered haulage this year. There were 12 powered haulage fatalities during the same period in 1994.
