METAL/NONMETAL MINE FATALITY - On October 4, 1995, a contractor foreman with 22 years of mining experience was killed at a sand and gravel operation. A cement take-up pulley weight for a conveyer was being lifted into position by a crane. To lift the weight, straps were attached to four pins that had been set into the cement weight. The employee was riding on the 3,000 pound weight as it was being lifted. One of the pins pulled out of the weight causing the employee to lose his balance and fall 35 feet to the ground. He died from sustained injuries. This is the 36th fatality reported in calendar year 1995 in the metal and nonmetal mining industries. As of this date in 1994, there were 31 fatalities reported in these industries. This death is the sixth fatality classified as machinery this year.
