MSHA - Fatal Alert Bulletin

METAL/NONMETAL MINE FATALITY - On February 9, 1996, a refractory worker (contractor employee) with three years of mining experience was killed at a limestone quarry. Four contractor employees were removing a 9-foot long section of casted liner from the 12-foot diameter rotary kiln. They had removed a key strip from the bottom of the liner and were in the process of chipping out the sides when the top section of the liner collapsed. One employee died from crushing injuries. The other three were injured. This is the fourth fatality reported in calendar year 1996 in the metal and nonmetal mining industries. As of this date in 1995, there were six fatalities reported in these industries. This death is the first fatality classified as falling/sliding material this year. There was also one falling/sliding material fatality during the same period in 1995.
