METAL/NONMETAL MINE FATALITY - On March 13, 1998, a 63-year old truck driver (contractor employee)  with 7 years of experience hauling to this facility, was fatally injured at a limestone quarry. He had dumped fly ash from one of the tandem trailers and was disconnecting the trailers preparatory to dumping the second one. The unit drifted and jack-knifed, pinning him between the two trailers.

Always set the parking brake prior to leaving the cab when operating mobile equipment.

When mobile equipment is parked on a grade, the wheels should be chocked or turned into the embankment.

This is the twelfth fatality reported in calendar year 1998 in the metal and nonmetal mining industries. As of this date in 1997, there were 16 fatalities reported in these industries. This is the third fatality classified as Powered Haulage in 1998. There were 9 Powered Haulage fatalities in the same period in 1997.
