• Machinery should be de-energized or moving machine parts should be guarded to protect persons from contact that could cause injury.

• Mine operators should establish procedures that assess all work tasks to identify and promptly correct all hazards.

This is the 27th fatality reported in calendar year 2000 in the metal and nonmetal mining industries. As of this date in 1999, there were 22 fatalities reported in these industries. This is the fifth fatality classified as Slip/Fall of Person in 2000. There was one Slip/Fall of Person fatality in the same period in 1999.
