METAL/NONMETAL MINE FATALITY - On August 9, 2000, a 42-year-old forklift operator (contractor employee) with 4 years mining experience was fatally injured at a dimension stone quarry. The victim was moving a forklift from the quarry to the shop when the forklift overturned. The forklift came to rest on its side and the victim was pinned in the cab.

Best Practices

  • Forklifts should be operated with the load/forks tilted back.

  • Forks should not be raised or lowered en route except for minor adjustments.

  • Operating speeds should be consistent with conditions of roadways, grades and the type of equipment used.

  • Operators of self-propelled equipment shall maintain control of the equipment while it is in motion.

This is the 31st fatality reported in calendar year 2000 in the metal and nonmetal mining industries. (On 8/8/2000, the Fatality Review Committee determined that fatal no. 8 on 2/21/2000 was not chargeable). As of this date in 1999, there were 33 fatalities reported in these industries. This is the 14th fatality classified as Powered Haulage in 2000. There were 12 Powered Haulage fatalities in the same period in 1999.
