• Persons should stay clear of all shaft openings whenever conveyances/buckets are in motion.
  
  
• Warning systems should be installed to alert employees that moving buckets are nearing or passing through galloways.

This is the 34^th fatality reported in calendar year 2000 in the metal and nonmetal mining industries. (On 8/8/2000, the Fatality Review Committee determined that fatal no. 8 on 2/21/2000 was not chargeable). As of this date in 1999, there were 34 fatalities reported in these industries. This is the first fatality classified as Hoisting in 2000. There were no Hoisting fatalities in the same period in 1999.
