METAL/NONMETAL MINE FATALITY - On April 25, 2001, a 39-year-old front-end loader operator with 2 years mining experience was fatally injured at a sand and gravel operation while helping to assemble the plant. A front-end loader and a chain were being used to raise a conveyor. The rigging failed and the conveyor fell, striking the victim.

This is the 12th fatality reported in calendar year 2001 in the metal and nonmetal mining industries. As of this date in 2000, there were 14 fatalities reported in these industries. This is the third fatality classified as Powered Haulage in 2001. There were eight Powered Haulage fatalities in the same period in 2000.
