• Personnel should be trained in safe work procedures.
• Feed and discharge controls should be de-energized, locked-out, and tagged prior to accessing bins and hoppers.
• Persons entering bins should wear a safety belt and line and a second person should be designated to tend the lifeline.

This is the 11^th fatality reported in calendar year 2001 in the metal and nonmetal mining industries. As of this date in 2000, there were 14 fatalities reported in these industries. This is the fourth fatality classified as Slip/Fall of Person in 2001. There were two Slip/Fall of Person fatalities in the same period in 2000.
