COAL MINE FATALITY - On Friday July 7, 2006, at approximately 8:05 a.m., a 35-year old Laborer/Millwright with 3 years mining experience was fatally injured while operating a man-lift during construction of a preparation plant. While repositioning the man-lift, the operator was pinned between an elevated metal cable tray and the handrail over the control panel of the platform.

Best Practices

Conduct pre-operational checks on all equipment. All defects affecting safety should be reported to a responsible person for correction.
Remove any malfunctioning equipment from service immediately.
Always look in the direction of movement and exercise caution in close work areas.
Do not use tram functions to position platform close to obstacles. Use boom function instead.
Do not use high speed drive when in restricted or close quarters.

	Submit your own suggestion for a remedy to prevent this type of accident in the future. Please specify if you wish your submission to be anonymous or whether your name may be used. Please include the year of the fatality and the fatality number.

This is the 34^th fatality reported during calendar year 2006 in the coal mining industry. As of this date in 2005, there were nine fatalities reported in coal mining. This is the third fatality classified as Machinery in 2006. There were no Machinery fatalities at this time in 2005.
