METAL/NONMETAL MINE FATALITY - On June 19, 2007, a 30 year-old underground blaster, with 4½ years experience, was fatally injured at an underground gold mine. The victim was traveling in a 2 cubic-yard Load-Haul-Dump (LHD) unit along a drift above a backfilled, previously mined stope. The LHD fell through the floor of the drift in a void that had developed in the backfill. The victim was recovered on July 2, 2007.

Best Practices

Supervisors or other designated persons should examine underground haulageways and travelways for changing ground conditions.
Design and utilize mining methods that will maintain stability and control the ground in places where persons work or travel.

	Submit your own suggestion for a remedy to prevent this type of accident in the future. Please specify if you wish your submission to be anonymous or whether your name may be used. Please include the year of the fatality and the fatality number.

This is the 14^th fatality reported in calendar year 2007 in the metal and nonmetal mining industries. As of this date in 2006, there were 14 fatalities reported in these industries. This is the 3^rd Powered Haulage fatality in 2007. There were 2 Powered Haulage fatalities in the same period in 2006.
