METAL/NONMETAL MINE FATALITY - On July 24, 2007, a 57 year-old superintendent with 27 years experience was fatally injured at a sand and gravel operation. The victim was driving a haul truck that struck a guard rail on a bridge and landed in a river below.

Best Practices
  • Ensure that pre-operational checks are conducted to identify and correct any defects that may affect the safe operation of self propelled mobile equipment.
  • Ensure that adequate berms of mid-axle height are provided for roadways where a drop off exists.
  • Ensure that widths of roadways are appropriate to safely accommodate the widest vehicles traveling over them.
  • When approaching and before proceeding on narrow roadways, reduce speed and proceed slowly.

This is the 18th fatality reported in calendar year 2007 in the metal and nonmetal mining industries. As of this date in 2006, there were 17 fatalities reported in these industries. This is the 5th Powered Haulage fatality in 2007. There were 6 Powered Haulage fatalities in the same period in 2006.
