District 5 - Coal Mine Safety and Health
On April 17, 2006, a non-fatal falling material accident occurred at a surface mine. During augering operations, after a heavy rainfall, a section of highwall fell causing serious damage to the Salem dual head auger. Two employees in the machine cab observed material falling from the highwall and immediately exited the cab without injury.
- Always have a certified person conduct a thorough pre-shift and on-shift examination of the highwall at augering operations.
- Inspect highwalls frequently during any augering operation conducted during/after a heavy rainfall.
- Provide auger machines exposed to highwall hazards with heavy gauge screen strong enough to prevent injuries to workmen from falling material.
