A Near Miss


Overturned Flatbed Truck

On December 16, 1999, a parts clerk was descending a 16 percent grade on a mine haulage road in a flatbed truck. The truck's transmission slipped out of gear, and the operator was unable to re-engage the transmission. He immediately attempted to brake and discovered that the truck's brakes were completely inoperative. In an attempt to slow the truck, he steered into and along an embankment on the roadside. After traveling along the embankment for approximately 500 feet, the truck overturned onto the edge of the roadway. The cab of the vehicle was crushed to the top of the seat back. The driver was hospitalized with broken ribs.
