Conduct self assessment

Obtain and put on PPE

Examine self-rescuer
Obtain and fill water cooler with water
Walk to production shaft
Assure the man cage to be spotted
Assure horn is sounded prior to opening shaft gate
Open shaft gate
Open the man cage door
Enter man cage
Close the shaft gate
Close man cage door
Ride man cage down to the nine hundred foot level after signal is given to hoist operator
Assure that the horn is sounded prior to opening the man cage door
Open man cage door
Open shaft gate
Assure skip is spotted at landing prior to stepping off
Exit man cage
Close man cage door
Close shaft gate
Release the man cage
Enter the air-lock
Close air-lock doors
Open inside air-lock door and exit
Close air lock after last person has exited the air-lock
Walk to transportation
Travel to sixteen hundred foot level shop
Save lunch in lunch room
Attend pre-shift line up with supervisor Priority 2

Where is the next face
Discuss any maintenance issues
2. Work place inspection
Check atmospheric condition
Smell the air for any unusual odors
Smell of rotten eggs would indicate the presence of hydrogen sulfide gas
Observe the changes in lighting
Amber color would indicate high nitrogen dioxide concentration
Inspect ground conditions for loose scales Priority 3
Back
Face
Ribs
If any loose ground is found report to supervisor, communicate with scale operator about scales to be removed. back under cutter out if needed
Inspect floor for slip trip and fall hazard Priority 3
If any unsafe condition is found contact supervisor and or correct unsafe condition
Check travel route for under cutter
Inspect for loose and deep soft salt
Loose deep salt could cause the under cutter to get stuck and or will cause equipment damage.
Assure no other equipment will be blocked by cable when under cutter enters face area.
Notify supervisor to have equipment moved.
Fill out work place inspection form immediately after work place inspection is completed
Responsibility of the operator to complete form. if form is not completed the employee would be disciplined
3. Conduct pre-shift inspection
Conduct a general walk around of under cutter
Check to assure equipment is not tagged out of service
If equipment is tagged out notify supervisor
Look for any unusual/ unsafe condition around and under the under cutter
Look for large puddles of fluid on floor , around and under cutter
Check electrical cable and clamp
Never handle energized cables
Use cable hook or attached rope if cable has to be handle
Never try to attach a rope to an energized cable
Insulated straining clamp must be secured to the cutter cable and cutter with chain
Ensure slack is provided between the attached clamp and plug
Ensure chain from insulated clamp is attached to the cutter
Walk the length of the cable from the cutter to the transformer looking for nicks, cuts and bare wires
Follow lock out tag out procedure if cable is damaged and notify supervisor
Plug cutter cable into transformer if disconnected
Ensure breaker is in off position
Lift receptacle cover
Raise the plug and engage the hooks onto the holding bar
Ensure proper lifting procedures are used
Rotate plug down into position
Apply downward pressure to assure plug is properly seated
Listen for clicking sound of latching mechanism and or try to raise rear of plug
Do not energize cutter cable until after checking GM 200
Check labels on plug and breaker
Ensure breaker and plug is properly identified
Locate and use magnetic labels if not properly identified
Remove proper magnetic label from door
Place on correct cutter breaker
Ensure cable is secured to transformer with attached rope

Push test button
Turn breaker back on if breaker trips
Notify supervisor If breaker does not trip
Lock and tag out if breaker does not trip
Check ground trip system
Push test button
Turn breaker back on if breaker trips
Notify supervisor If breaker does not trip
Lock and tag out if breaker does not trip
Energize cable if needed Priority 3
Stand off to the right side of breaker when energizing
Raise breaker handle to energize
Check cutter head oil level
Remove top plug to assure proper oil level
Plug is located front center of the head
Notify supervisor and or serviceman if no oil is present
Check head cover plate
Assure bolts are in place and tight
Notify supervisor if bolts are missing or loose
Check cutting bar, chain, and Dovetail and bolts
Look for missing rivets and damage wear strips on the cutting bar
Check for excessive play on the hanger bar and dove tail
Excessive play is more than a one inch gap between hanger bar and dove tail
Notify supervisor of any defects
Check for broken or missing bits on the chain
Cutter operator will install new bits after completing pre-shift inspection using proper bit changing procedure
Assure no more than five or more bits are broken off flush with the block or any two in a row
Assure cutter is tagged out and not be operated if this condition is found Priority 2
Notify supervisor if above condition is found
Check for loose chain
Assure there is no more than one inch play between the sprocket and end of the bar
Assure cutter is tagged out and not operated if this condition is found Priority 2
Notify supervisor if the above condition is found
Check exposed chain guides for excessive wear
Assure no sharp edges are visible on chain guide
Notify supervisor if above condition is found
Assure dove tail bolts are in place and tight
Check for cracks and metal fatigue on the dovetail
Metal fatigue can be identified by discolored metal
Check front U-joint and bolt
Assure by visual inspection that all bolts are in place and safety wire is in tact
Assure cutter is tagged out and not operated Priority 2
Notify supervisor if above condition is found
Check back U-joint and bolts
Assure by visual inspection that all bolts are in place and safety wire is in tact
Assure cutter is tagged out and not operated Priority 2
Notify supervisor if above condition is found
Check transmission oil level
Assure oil is in site glass
Located at bottom right side of transmission
Contact serviceman if oil is needed
Check water level
Assure water tank is full by opening cap on top of tank
Contact serviceman if water is needed
Check water hoses
Look for wetness, cracks around the hose
Check hydraulic oil level
Remove top plug to assure proper oil level
Plug is located between operators cab and left steering tire
Notify supervisor and or serviceman if no oil is present
Check all hydraulic hoses and fittings for oil leaks
Visually inspect for damaged, hoses loose fittings, frayed,obvious wear
Notify supervisor if leak is detected
Check control valve
Assure valve is not leaking hydraulic oil
Notify supervisor if leak is detected
Assure all levers are in place
Check steering cylinders,pins,mounts, and retainers
Assure pins, mounts and retainers are in place and no visible leak on steering cylinder
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check stab jack cylinders, pins, mounts, and retainers
Assure pins, mounts and retainers are in place and no visible leak on stab jack
Notify supervisor if defects are found
Check cutter electric motors
Assure guards are in place
Assure fan is on back of motor
Assure electrical connections are in tact
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check electrical boxes
Assure all wires are properly bushed where it enters and exit the box
Assure the doors are closed and latched
Assure there are no exterior damage to the box
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check fire extinguishers.(2) Priority 3
Assure pin is in place and retained
Assure it is fully charged
Check for cracks in the hose.
Assure monthly inspection is current
Assure the extinguisher is type ABC
Underground mine fires cause carbon monoxide gas and oxygen deficient atmosphere and can be explosive with right mixture of oxygen
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check lights
Assure front and rear lights are functional
Assure lights are properly mounted to the cutter frame.

Check wheel lug
Assure all are present and tight
Tag out if any defects that affect safety are found Priority 2

Check headache rack
Check for cracks in the welds
Tag out if any defects that affect safety are found. Priority 2
Notify supervisor of any defects that affect safety are found
Check housekeeping
Check for trash and remove if necessary
Check boom lift cylinders, pins, mounts and retainers
Assure pins, mounts and retainers are in place and no visible leak on boom lift cylinders
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check boom swing cylinders, pins, mounts and retainers
Assure pins, mounts and retainers are in place and no visible leak on boom swing cylinders
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check bar swing cylinders, pins, mounts and retainers
Assure pins, mounts and retainers are in place and no visible leak on bar swing cylinders
Tag out if any defects that affect safety are found Priority 2

Check boom roll guard bolts
Assure bolts are in place and tight
Tag out if any defects that affect safety are found Priority 2
Notify supervisor if any defects are found
Check tires
Assure tire is not peeling off of the rim
Check for missing chunks on the tires
Notify supervisor if any defects are found
Turn on hydraulic motor
Push green indicator button labeled pump start
Check bar tilts cylinder, pins, mounts, and retainers
Raise cutter head by lifting boom lift control lever
Tilt cylinder is located below the cutter head
Assure pins, mounts and retainers are in place and no visible leak on bar tilt cylinders
Tag out if any defects that affect safety are found Priority 2
Notify supervisor of any defects that affect safety are found
Check brakes
Engage brake by pushing brake control lever down to set brakes
Apply brakes and attempt movement of cutter to assure brakes are holding
Push down on slow tram control levers
Notify supervisor if brakes are not holding
Check tram motor and chain
Release brakes and push down on tram control levers one at a time to assure operation of motors
Assure chain is in tact without excessive slack
Excessive slack is chain dragging on ground or rubbing the cutter frame
Tag out if any defects that effect operation of the tramming components are found Priority 2
Notify supervisor if any defects that effect operation of the tramming components are found
Check bug duster components.(2)
Assure bolts are in place and tight
Check for hydraulic fluid on floor
Push bug duster levers down to assure proper rotation
Ensure both augers are turning freely
Tag out if any defects that effect operation of the bug duster components are found Priority 2
Notify supervisor if any defects that effect operation of the bug duster components are found
Report any safety defects and immediately shut down and tag out and report to supervisor and all equipment defects or unusual conditions to supervisor Priority 3
Fill out pre-shift inspection form immediately after conducting pre-shift inspection
Responsibility of the operator to complete form. if form is not completed the employee would be disciplined
