    Excavator Blocking Issue
    Raising a hydraulic excavator to perform maintenance or repair introduces hazards to persons working around the machine. It has been found that repair shops have raised and blocked the entire machine and then lowered the bucket to the floor to provide stability.

    Two recent fires occurred that involved front-end loaders. These fires proved very dangerous to miners and fire fighters because the off-road tires exploded, resulting in a dangerous release of energy.

    Equipment Hazard Alert

    Atlas Copco LHD Loaders and Trucks - Service News No. 100624-972
    In a recent metal/nonmetal mine fatality, a mechanic was fatally injured while checking for a hydraulic leak on an Atlas Copco ST-8B Scooptram. While checking for a hydraulic leak, the victim leaned into the ejector plate relief port in the back of the loader bucket. The ejector plate of the bucket was retracted, crushing him against the back of the bucket.

    Bleeder Ventilation Hazards
    Obstructions such as roof falls and standing water in bleeder systems can severely restrict ventilation and prevent proper weekly examination to evaluate the effectiveness of the bleeder system. Mine explosions have been attributed to lack of adequate ventilation.
