Equipment Alert Posted
Loading and continuous-mining machines, since 1981, are required by MSHA to be equipped with load-locking valves in the boom and head lift cylinders. The load locking system installed must be maintained in proper working order. Machines that have no system or have systems that fail to support the machine structure in the raised position will be cited as not being in compliance with Title 30 Code of Federal Regulations Part 75.503, failure to maintain face equipment in permissible condition. |

The following incident took place on 23 September, 2008.
At 5:15 AM, Tuesday, September 23, a Dyno Nobel Transportation driver picked up Carbo*Prill from a storage bin at the Owensboro, KY Riverport Authority site. The Port Authority Loader saw "orange glowing embers" as he loaded the Carbo*Prill into the DNTI trailer. |

Safety Alert
On October 22, 2008, a dozer operator was fatally injured when the dozer he was operating
slipped and rolled sideways down a steep 80 foot incline coming to rest on its left side. This
was the fourth accident in five weeks where a dozer operator was either fatally injured or
suffered serious life threatening injuries. |

Remote control continuous mining machine operators continue to lose their lives while tramming these machines. There have been 31 fatalities involving remote control continuous mining machines since 1984, two of which have occurred in 2008. |
