A 21-year-old man standing on a cliff at a quarry in New Lexington, Ohio, lost his footing and, despite attempts to grab onto vines, short grass and trees, fell down the rocky cliff into the water below. He was pronounced dead at the scene. His body was recovered by officials from the local fire department and dive rescue team. (Source: MSHA)
