An owner of approved (permissible) equipment who desires to make modifications in such equipment shall apply in writing to make such modifications. The application, together with the plans of modifications, shall be filed with Approval and Certification Center, RR 1, Box 251, Industrial Park Road, Triadelphia, WV 26059. Any personal information (e-mail or otherwise) submitted is used for contact purposes only. This personal information is not used or distributed to any other agency or used for any other purpose. Persons are not required to respond to this collection of information unless it displays a currently valid OMB control number. |
