Within 30 days of the opening of a new mine or when there are any changes to the legal ownership structure for a mine, a mine operator must file a Legal Identification Report with MSHA. MSHA uses this information to properly identify persons charged with violating mandatory safety and health standards and in the assessment of civil penalties on those violations. The Office of the Solicitor uses the information to expedite service of documents upon the mine operator.

This form is not used to apply for a Mine ID; rather, it is for submitting an initial Legal ID Report or updating an existing Legal ID Report. Mine Safety and Health Enforcement
All information previously submitted remains in effect except where changes have been submitted. If the changes provided on this form affect other mines, a separate form must be filed for each Mine ID number.
