Plant – Cement – More than 600 feet of a metal canopy structure collapsed unexpectedly at a cement plant operation, injuring three miners. The new structure was being erected to serve as shelter for stockpile material from weather conditions.
• Always use fall protection where there is a hazard of falling.
• Consider using fall restraint devices to eliminate hazards of falling.
• Examine structures for indications of damaged or weakened structural elements.
• New structures should be designed and certified as appropriate by a registered professional engineer in accordance with local building codes and in conformance with accepted engineering practices.
