Surface – Sand and Gravel – On 3/13/2018, a front end loader operator was constructing an embankment to detour material away from the water supply.

Surface – Stone– On March 7, 2018, a welder was installing a caged ladder on a screening deck when the ladder suddenly fell, striking his hard hat.

Plant – Limestone – On 2/01/2018, the victim was cleaning up spillage near the dryer when his gloved right hand was pulled into the reciprocating feeder.  He received serious injur

On February 12, 2018, a plant operator was assisting another miner in removing a head pulley. While ascending a 12-foot step ladder, the victim’s foot slipped and he slid down the ladder.

On February 8, 2018, a plant operator was shoveling material from around a crusher when a rock fell from the Surge Bin deck above, striking the victim on the head.

Surface – Sandstone – On 01/18/2018, a miner was operating an excavator equipped with a rock breaker attachment to break up large boulders.  A rock penetrated the right side
