Phosphate/Surface – A miner operating a loaded articulated, haul truck drove through a berm and rolled the truck down the face of the dam. The miner was able to break out and crawl through the windshield and onto the hood to wait for help to arrive. The miner was taken to a local hospital with serious injuries.
