METAL/NONMETAL MINE FATALITY - On September 1, 2008, a 67 year-old contractor consultant with 46 years experience was fatally injured at a copper mine. The victim had been instructing persons making a belt splice. He fell through a 4-foot by 6-foot opening 28 feet to the floor below.

Best Practices

  • Establish and discuss safe work procedures. Identify and control all hazards to finish the job safely.
  • Train all persons to recognize and understand safe job procedures before beginning work.
  • Wear fall protection where there is a danger of falling.
  • Protect openings near travelways through which persons may fall by installing railings, barriers, or covers.
  • Keep temporary access opening covers or railings secured in place at all times when the opening is not being used.

This is the 15th fatality reported in calendar year 2008 in the metal and nonmetal mining industries. As of this date in 2007, there were 23 fatalities reported in these industries. This is the 3rd Fall of Person fatality in 2008. There were 3 Fall of Person fatalities in the same period in 2007.
