METAL/NONMETAL MINE FATALITY - On October 1, 2008, a 37 year-old front-end loader operator with 3 years of experience was fatally injured at a sand and gravel operation. The victim was traveling on a mine road in his personal vehicle when it traveled through a berm and into a body of water.

Best Practices

  • Maintain control of self-propelled mobile equipment while it is in motion.
  • Ensure that adequate berms of mid-axle height of the largest mobile equipment are provided for roadways where a drop off exists.
  • Construct berms with a steep inside slope and with materials that can be well compacted.
  • Reduce speed where visibility is limited.

Additional Information

This is the 16th fatality reported in calendar year 2008 in the metal and nonmetal mining industries. As of this date in 2007, there were 28 fatalities reported in these industries. This is the 4th Powered Haulage fatality in 2008. There were 7 Powered Haulage fatalities in the same period in 2007.
