COAL MINE FATALITY - On Friday, August 22, 2008, a 38 year old contract truck driver with three days experience was fatally injured on a coal haulage road. The truck struck the berm of the road near the crest of an average 5% grade road, at which time the driver either fell or jumped from the truck and was run over by the rear wheels.
Best Practices
- Conduct pre-operational checks to identify defects that may affect the safe operation of equipment before being placed into service.
- Observe all speed limits, traffic rules, and ensure that grades on haulage roads are appropriate for haulage equipment being used.
- Never operate a truck or other mobile equipment without using a seat belt.
- Know the truck's capabilities, operating ranges, load-limits and safety features.
- Always select the proper gear and downshift well in advance of descending the grade.
- Conduct thorough, in depth task training to cover potential hazards.
Additional Information
This is the 19th fatality reported in calendar year 2008 for coal mining. As of this date in 2007, there were 24 fatalities for coal mines. This is the 7th Powered Haulage fatality and 2nd Surface Powered Haulage fatality in 2008. There was one Powered Haulage fatality in the same period in 2007.
