Preliminary Report Overview

Guerra and a coworker were replacing a snub pulley on an elevated radial stacker conveyor. Guerra was standing on a walkway on one side of the conveyor and his coworker was standing on a walkway on the other side of the conveyor. They were attempting to maneuver the pulley into position to bolt it in place when the victim fell 16 feet to the ground below. He was hospitalized and died on February 26, 2012.
