Preliminary Report Overview

The victim entered a highwall miner entry in the Fire Clay coal seam to retrieve the right side cutter head chain for the Superior Highwall Miner Machine, S/N 61. Approximately 37 feet into the 9.5 feet wide entry a rock 8 feet in width by 6 feet in length and 16 inches thick struck the victim. Two miners rushed to free the victim and extracted the victim to the entrance of the highwall miner entry. The victim was transported by ambulance to a hospital and later died. MSHA accident investigators are conducting an investigation of tha accident.
