The mining operator must follow all applicable standards outlined in 30 CFR Chapter 1. Hazardous condition complaints may be reported by calling 1- 800-746-1553, or by using the online filing links below.

You can report your complaint anonymously using this system.

If an accident, injury or illness occurs at or in conjunction with activity at a mine, mine operators are required to report the circumstances of the incident to MSHA using Form 7000-1. File online electronically here.
