MSHA Frequently Asked Questions

Injury reporting

What are the mine operators’ requirements to report an accident, injury, or illness?

When an accident, injury, or illness occurs at or in conjunction with activity at a mine, mine operators are required to report the circumstances of the incident to MSHA using Form 7000-1. You can file online using your EGov account.

Where can I find an accident report for an accident that occurred before 2004?

MSHA’s Digital Library collections contains full text fatality and accident reports of over 30,000 Agency investigations dating back to 1840 and research materials produced by the Bureau of Mines, MESA and MSHA since 1910. These items, as well as photos and videos, can be searched and browsed here

For help to find reports that have not been digitalized, contact MSHA’s Academy Library at 304-256-3372, or email your question to MSHALibrary@dol.gov

How can I learn about MSHA reporting requirements?
